Un mode de réalisation du “ plan de soin
gériatrique ” tel que défini par l’arrêté royal au
sein d’un hôpital général : définition du patient
gériatrique, son dépistage, son suivi et la
transmission des informations. L’évaluation des
premiers résultats montre l’importance de la prise
en compte des aspects biographiques et socioéconomiques,
l’indispensable optimisation d’un
screening précoce et la communication entre
soignants pour améliorer le trajet de soin de ces
patients.

Abstract in English:
A concrete realisation of the “ Geriatric
assessment and management program ” as
described in the arrêté royal within a general
hospital : definition of the geriatric patient, his
detection, his follow-up and the transmission of
patient information. The analysis of the first
results indicates the importance of the biographic
and socio-economics factors. The optimisation of
an early screening and the communication
between caregivers are key aspects to enhance
the care of this patients.
